Since the work morphed in online slang to refer to vegan girly men, that wear skinny jeans, have man-buns, and are keyboard warriors but chickenshits in real life. Unless they're stabbing you in the back and whining to Human Resources about their safe spaces, or in an armored mob. The left had to twist the meaning.
So they pretended, or worse, gullibly believe, it was used by White Supremacists, as a slur. Because everything the left doesn't like is racism/Hitler. In truth, it was just the anti-left (often right-leaning folks) hitting back at the Politically Correct wimps, that need safe spaces.
From there it morphed to mean Cuckservatives (RINOs) or not real conservatives. Again, the backstabber, not-real-men connotations. And that was picked up by those that weren't establishment right. And then it was used by a few that were alt-Right... and some of the alt-right are white nationalists, but many are just pretending to be to piss off the far-left snowflakes. So there is some validity that it evolved towards being used by racist Alt-Right... but the term is a lot broader than that. And like the OK hand gesture being morphed into standing for "White Power" the left completely overstates things or misses when they're being mocked.

It's about a loaner who lives with his mom, unfit for military service due to his history of mental instability, who retreats into far-right extremist internet groups and his guns. Basically all the far left stereotypes aboout right wingers, and making them the cuck. (inverting the normal usage).[1] |
BTW: The gesture of putting a peace sign behind someone's head in photos (or behind their back), originated with implying either his wife was two-timing him, or demon... or inference to both. Also, the gesture for "Hookem Horns" (UT) is used for cuckolds.
